  I have read, understand and agree to comply with the Club's Facility Hire Terms and Conditions.

• PUBLIC LIABILITY INSURANCE
  
  I hold a Public Liability Insurance Policy issued by a reputable insurer with cover of at least $20 million for each individual claim. The policy covers covers the booking period/s specified in the booking application. I will email a valid certificate of currency for the policy to vice.president@loganhockey.au within 2 business days of the application. I agree that the application may be rejected or cancelled by the Club if this condition is not satisfied.
• HIRE FEES AND CHARGES
  
  I agree to pay the Club's hire fees and changes for each booking date not less than 48 hours prior to the booking date. I accept the hire fees and charges published on the Club's website.
